APPLICATION
An application form is available in PDF format for you to download and print. The completed form is to be signed by the student, and the advisor for the student's major. Students who have not yet been admitted to a major should obtain the signature of their academic advisor in their college or DUS.
After the form is completed and the advisor's signature is obtained, it should be taken to the program director of the Environmental Inquiry minor, Dr. Denice Wardrop(contact information below), who will provide a copy to the student, retain a copy, and send a copy to the student's advisor for entry into University records ( ISIS).
